Registration forms can be downloaded here. The event will begin with registration at 11:30 a.m., continue with lunch at noon, and shotgun start to the tournament will begin at 1:00 p.m. Snacks and food will be available after the tournament.

Players may sign up as a team of four ($600/team) or as individuals ($150/player), and a team scramble format will be used with a max score per hole of a bogey. Hole partnerships are also available in two different ways – a hole partnership paired with a participating golfing foursome ($650) or a standalone hole partnership with no golfing ($150). Players will play to par on each hole, with the assigned par being the worst score available to each player. Team handicaps and final scores will be determined by Lakewood Golf Course Officials.

Participation fees for all players will include 18 holes of golf, use of a cart, lunch, and a goodie bag. Designated specialty holes will also have additional prizes: Closest to the Pin and Longest Drive That Stays in the Fairway, will win prizes.

Door prizes will be available in addition to a raffle, with prizes including: Tickets to Jazz Fest, a two-night stay at Hotel Monteleone, Edwin Watts gift certificates, Pat O’Briens gift cards, and more.

Vendor partnerships are also available. Vendors, players, and partners may contact Randy McCloskey at Randy.McCloskey@fgno.com or at (504) 948-1149 for more information.
